Research interest:

My main research interest is in cancer epidemiology, cancer biology, cancer biomarkers and diagnostics, and the diagnostic and prognostic application of cervicovaginal microbiota for cervical cancer prevention and control. I'm very curious in understanding the molecular epidemiology of cervical cancer and mechanisms of carcinogenesis.

II. Research statement

My main research interests include the field of epidemiological research with a focus on agent-based simulations to enable micromanagement in specific regions. This type of simulation allows to take into account local conditions such as transport systems, population behavioral patterns and subpopulations.  In particular, I am interested in the effects of co-infections for the healthcare system and the associated risks.

Within these possibilities I work together with the cities of Cape Town and Buenos Aires to advise them on political decisions.

 

III. Research project within the Global Health AG

Agent-Based Simulations in the global South

COVID-19 has spread from China to Europe and the United States and has become a global pandemic. In the countries of the Global South, effective local countermeasures continue to be discussed due to the often weaker socio-economic options and healthcare systems. Large-scale socio-economic and traffic survey data and detailed agent-based simulations of urban transport are combined to analyze the spread of COVID-19 in regional models. Through this micromanagement individualized countermeasure scenarios can then be analyzed.

Hello, I'm Antonio Torres Reyes, I hold a Master's degree in Geography, Environment, and Territorial Planning from the University of Havana. My previous research delved into "The Vulnerability to COVID-19 in Havana City" through a health areas approach, and I also explored the causes and effects of heat islands in Havana during the summer of 2017 for my Bachelor's degree from the same institution.

I've also been part of the Complex System Center at the Faculty of Physics, focusing on mobility and epidemiology research. Additionally, I'm well-versed in LaTeX for document preparation.

Currently, I'm pursuing a Ph.D. in the Global Health Group since the Winter semester of 2022. My ongoing research project focuses on estimating health-related variables in urban areas, particularly informal settlements. This project aims to provide essential data to inform governmental and health-related decision-making. While traditional estimations rely on resource-intensive surveys, our innovative approach harnesses technologies like remote sensing and AI to offer cost-effective alternatives. By merging rapid remote sensing measurements with deep learning's ability to extract meaningful patterns from images, we aim to provide accurate estimates of critical environmental factors impacting human health.

Presently, our research spans both formal and informal areas in Lima, Peru, with data collected from around 800 households using a spatial sampling strategy. Upholding ethical standards, we ensure informed consent and data anonymization are integral components of our methodology. This integration of remote sensing and AI holds the potential to shed light on the influence of environmental factors on population health in informal settlements, offering valuable insights for informed decision-making.

I will use mixed method, both quasiexperimental study and phenomenological explorative qualitative study design will be employed sequentially. Two population proportion formulas will be used to calculate the sample size. I expect Whole Person Care to be effective model to improve cancer care and treatment shared decision making.

 

BASELINE ASSESSMENT AND KEY INTERVENTIONS TO IMPROVE FOLLOW-UP ADHERENCE, RECURRENCE RATES AND ASSOCIATED FACTORS AFTER PRECANCEROUS CERVICAL LESION TREATMENT IN RURAL ETHIOPIA

b) evaluate different strategies to improve the follow-up adherence level after women treated for pre-CC lesions.

I will use formative assessment and observational studies to determine the follow-up adherence, recurrence rates, and associated factors among women treated with pre-cervical cancerous lesions. Then, a pragmatic randomized control trial will be used to evaluate key intervention strategies. I expect to provide feasible and effective interventions that improve the follow up level.

Improving the timely diagnoses and treatment of breast cancer in secondary level hospitals of Ethiopia

I aim to identify barriers and enablers of pathways to breast cancer care and implementing clinical breast examination and navigation of symptomatic women for diagnostic work up and linkage to treatment in Ethiopia. I will use mixed study designs (Cross-sectional, qualitative, cohort study design, Stepped-wedge cluster randomized trial, Systematic review and meta-analysis) to address the objectives of the study. The Binary logistic and Cox-proportional hazard regression models will be fitted to identify predictors of each outcome. I expect this study will provide evidences on the barriers and enablers of pathways to breast cancer care (personal, environmental and health system factors), and identify areas of intervention to improve early diagnosis of breast cancer in Ethiopia.

Feasibility assessment on improving early detection of breast abnormality with coordinated referral application support in primary health care setting in Ethiopia

I aim at assessing feasibility of using digital application to enhance the referral path way and follow up of women who undergo clinical breast examination at primary health care level in Ethiopia. Baseline Preliminary assesment and qualitative study will be conducted to understand breast abnormalities and out come of patients with breast related abnormality and identify potential barriers and facillitators for use of patient registry app. I will use cluster randomized feasibility trial to evaluate the intervention. Participants that will be followed using the patient registry application will be the intervention arm and participants that will be on routine care at the health facilities will be a control arm. Improved referral path way will be the primary outcome of the study.

I expect follow up and early detection of breast cancer among patients with breast abnormality will be improved and intercommunication between health facilities will be enhanced. I expect to understand more about the barriers for follow up and referral of patients with breast abnormality at the context of primary health care level.

Comprehensive palliative care model for women with cancer in Ethiopia

PhD candidate fellow in the Network for Oncology Research in Sub-Saharan Africa

I aim to design comprehensive palliative care model with continuum of care that links female cancer patients to resources at household, community and facility levels. The study will be a three year and three phased PhD project adopts an exploratory sequential mixed method design. The first phase will explore and identify current practices, needs, and challenges of palliative care and opportunities for comprehensive care provision among women with cancer. This phase will be guided by socio-ecological model to discern practices, challenges and opportunities at different levels. Based on the outcome of the first phase, in the second phase comprehensive palliative care model will be designed. This phase will be guided by world health organization stepwise framework, African experiences and Ethiopian national guideline. The final phase aims to validate the model using the Delphi technique, expert review workshop, and community dialogue. Finally, I expect to have contextually sound, feasible and acceptable comprehensive palliative care model with continuum of care, policy brief focusing on the way forward to build up comprehensive and continuum of care elements, and three published research articles.

The main objective of this research project is to assess time interval in Cervical cancer (CC) pathways, from suspicious CC to diagnosis and treatment, evaluate how clinical, socio-demographic, and treatment factors affect delays throughout patients’ clinical pathways in Ethiopia. A retrospective study method will be implemented. This research project will provide information and improve knowledge on pathways from suspicious CC to symptom onset, disease presentation and treatment for effective cervical cancer screening interventions and for early detection and treatment in Ethiopia.

I have been teaching health economics courses in the university since 2014 G.C. During these times, I advised many MPH students who did their researches on health economics areas also I have been participating in different health economics researches as assigned by the university and with my initiation.  Among these researches, the researches that I am doing currently for my PhD dissertation, entitled on the Economic burden of breast cancer that consists of three researches namely “Costs of breast cancer treatment in Addis Ababa, Ethiopia”, “Willingness and ability to pay for breast cancer treatment in Addis Ababa, Ethiopia and “Cost effectiveness of breast cancer in Addis Ababa, Ethiopia” can be cited.  Also, I produced one chapter on the economic burden of Non-Communicable Diseases (NCDs) to the module of NCDs.

With respect to work experience, I am working as freelance consultant since January 2010 to date.  During these times I achieved many commendable results but for this profile the most recent achievements are the following,

  1. I am doing “Public Expenditure Review of Health for Ethiopia at Sub-National Level”.  This assignment is the first in its kind in Africa, which is financed by World Bank but owned by FDRE-Ministry of Health but the organization that I am working with is IPE Global International consulting firm.
  2. I completed very recently on sustainability strategy and planning of Neglected Tropical Diseases (NTDs) for both international and national NTDs program implementers in Ethiopia.
  3. Last year I developed “Monitoring and Evaluation, Capacity Building, Sustainability and Phase-Out Strategies for Rights-Based Approach to Adolescent and Youth Development in Ethiopia Joint Programme Phase III (2018-2021).  The client was UNICEF-Ethiopia but the organisation that I affiliated with was ABH consulting firm.

Evaluation of the status of psychosocial service integration and implementation for people with cancer in Ethiopia

Evidence showed that psychological problem is a frequent but underrecognized and undertreated condition among cancer patients, which causes amplification of physical symptoms, increased functional impairment, and poor treatment adherence, whereas psychological interventions have been shown to increase adherence to treatment and quality of life by decreasing psychological symptoms like anxiety and depression. Despite, the importance of psychosocial support, studies are lacking in supporting these reports in Ethiopia. Hence, the main aim of this study is to explore challenges and opportunities in the implementation of psychosocial services in 12 hospitals in Ethiopia, to analyze the two-year survival of patients with cancer, and to estimate the cost and design integration guidelines for psychosocial services in routine cancer care setting in Ethiopia. To evaluate the implementation of psychosocial service provision in routine cancer care settings in Ethiopia. A mixed-method study approach will be used in this study. A qualitative study will be conducted to assess the facilitators and barriers of the formerly designed and integrated psychosocial support. Focus group discussions (FGDs), In-depth interviews (IDIs), and key informant interviews (KIIs) will be conducted with community representatives, health professionals, and patients, to assess the facilitators and barriers of the integrated psychosocial service. Based on the result of the qualitative study, the lacking component of psychosocial support will be added and integration guidelines will be designed. A two-year survival of patients with cancer will be analyzed, further, the cost of the integrated psychosocial service will be estimated. The qualitative data will be analyzed using content analysis, while survival will be analyzed by calculating overall survival rates and diseasespecific survival rates using the Kaplan-Meier method. The survival curves will be compared using the log-rank test, and p values will be calculated accordingly, whereas, cost estimation will be done by considering costs for each component such as one-time costs (infrastructure setup) and ongoing costs (personnel salaries or training). The assessment of the current status of psychosocial service integration and implementation for people with cancer will enable healthcare providers to ensure that these services are meeting the needs of their patients and are making a positive impact on patient outcomes.

Wondimu Ayele is Statistician in background and specialized in public health track of biostatistics and health informatics. He is an Assistant Professor for Biostatics and Health Informatics and Health Informatics Capacity Building and Mentorship Project Director at School of Public Health, College of Health Sciences in Addis Ababa University. He has worked at Federal Ministry of Health, Addis Ababa University, World Bank and World Health Organization for more than ten years. He joined the SPH as an academic staff in July 2013 and has been engaged in teaching biostatistics, Health Informatics and Research Method courses for both undergraduate medical and postgraduate public health and biomedical students at the College of Health Sciences. His is interested in data science, non-communicable diseases research and implementation science research to strengthen health information system in low income countries. Wondimu, Participant redesign of health information management system in Ethiopia. he has remarkable experience epidemiological research design, in-depth analysis and operational research, infectious diseases transmission modelling and predication. He participated several survey protocol design and data management and analysis such as Malaria Indicator survey, Health facility mapping, district profile preparation, EPI cluster survey, Health services provision assessment (SPA), SARA, Demographic and Health survey and also several health and health-related baseline, midterm and outcome evaluation and research. He has published over 19 peer-reviewed papers in reputed international journals so far. Wondimu is a PhD Fellow at Institute of Medicine, Epidemiology, Biostatistics and Informatics, Martin Luther University, Germany. His research work entitled “Modelling Breast Cancer burden estimation and predication in Ethiopia using Population based longitudinal data”
